Action Plan Logic Model Form
Coalitions will submit this form along with their Action Plan grant request to identify a specific problem in their community, what activities and resources are needed to correct this problem, and what measurable outcomes or impacts to expect.
Action Plan Progress Report - On-line Form
Coalitions will use this on-line form to report whether their project was completed, how many people were involved, whether it met your Action Plan objectives and to describe the most significant accomplishment of the project. To receive a copy of the form as a Microsoft Word document click here.
Action Plan Reimbursement Form
Coalitions will use this form to submit reimbursement claims for expenses approved in their quarterly Action Plans. Form must be returned with proper documentation of the expenses claimed.

Coalitions submit this form with their Action Plan to detail projected expenses.

This 9-page guide in pdf format provides an overview of how to initiate anti-drug activities in schools at all grade levels. An anti-drug campaign is especially useful to kick-off a UNITE Club or special celebration.
Drug-Free Pledge Certificate
A printable certificate that can be presented to youth who pledge they will stay drug free. May be used in conjunction with any anti-drug school activity.

This on-line form is used to submit information about an upcoming anti-drug event that you would like publicized on UNITE’s website under “Upcoming Events” and/or placed on the homepage of the county where the event will take place.
